Medical-device audit readiness: what inspectors need to see

Audit readiness is not a last-minute document collection exercise. The strongest systems make it easy to trace a requirement to the process, the record, the metric, and the corrective action that keeps the process controlled.

Inspection readiness starts with normal operations

FDA’s QMSR took effect on February 2, 2026, and FDA began using the inspection process described in Compliance Program 7382.850. Regardless of the inspection format, a practical readiness program should make it possible to demonstrate how the quality system functions in day-to-day manufacturing.

Best test: can the organization explain the same process consistently through its procedure, operator practice, manufacturing records, validation evidence, metrics, and corrective-action history?

Six areas worth stress-testing

1. Documented process

Controlled procedures and work instructions reflect current manufacturing practice, responsibilities, acceptance criteria, and records.

2. Training

Personnel are trained on the current revision and can demonstrate the process they are expected to perform.

3. Validation

Validation evidence supports processes requiring validation and connects to equipment, parameters, methods, and routine production controls.

4. Supplier controls

Critical suppliers are qualified, monitored, and subject to clear expectations for changes, investigations, and corrective action.

5. Nonconformance & CAPA

Investigations reach defensible root causes and corrective actions include objective evidence of effectiveness.

6. Management evidence

Quality data is reviewed, trends are visible, and management decisions connect to measurable risks and follow-up actions.

Do not build an “inspection version” of the quality system

If records must be recreated, reconciled, or explained differently for an inspection, the real operating system may not be inspection-ready. Readiness should come from maintaining complete records as work is performed.

Trace one issue end to end

A useful internal exercise is to select a real nonconformance or supplier defect and follow it through the entire system. Can the team show containment, investigation, root cause, corrective action, change control, implementation, and effectiveness evidence? Can the same evidence be tied back to the relevant risk and process controls?

Look for disconnects between departments

Many audit problems occur at handoffs: engineering changes a process but training is not updated; supplier quality closes a SCAR without checking internal impact; manufacturing uses a workaround not reflected in the work instruction; quality trends a defect but operations does not have an escalation threshold.

Audit-readiness work should therefore test interfaces between functions rather than reviewing each procedure in isolation.

Use objective evidence, not confidence

Statements such as “we always do this” are weak without supporting records. A stronger system makes objective evidence easy to retrieve: approved instructions, training records, validation protocols, acceptance data, supplier records, deviation history, CAPA effectiveness checks, and change-control evidence.

A practical readiness review

  • Select high-risk manufacturing and supplier processes.
  • Compare controlled documents with actual execution.
  • Check whether validation assumptions remain true.
  • Trace recent nonconformances through root cause and effectiveness.
  • Review supplier changes and recurring defects.
  • Confirm that metrics trigger action when thresholds are exceeded.
  • Verify that records are complete, current, and retrievable.
